Service of AGREEMENT Dorcas Dezines referred to as CONSULTANT and name on the below agreement which will be referred to as EMPLOYER, agree to the below conditions: Dorcas Dezines shall provide consulting services regarding the below listed EMPLOYER beginning on (date below) and ending on (date below), unless earlier terminated as is provided for herein. EMPLOYER agrees that it shall reasonably cooperate with CONSULTANT in the provision of information required by the CONSULTANT. If timelines of work is not submitted within the time of agreement, Dorcas Dezines takes no responsibility for the time of completion of work. Dorcas Dezines shall provide the best efforts to the project consistent with the stated goals of the EMPLOYER. However, Dorcas Dezines cannot guarantee any particular result due to the EMPLOYER'S ultimate control of the project(s) that has not been submitted in advance. All work provided to Dorcas Dezines by the EMPLOYEE must be proofed and ready to be published. Dorcas Dezines must have a layout of all work before processing work, unless other wise stated by both parties. If work has no layout and Dorcas Dezines has to layout work, Dorcas Dezines will charge an additional fee for hire depending on work assignment. All work must be electronically transmitted, via email. Any work that is submitted via hard copy will be charged an additional fee (except for web casting and web radio). Dorcas Dezines handles all billing statements via email. All clients must have a working email address. Any postal mail will occur an additional shipping and handling. Dorcas Dezines refund policy for printing publications, shall be as followed: Dorcas Dezines will replace any work that has been damaged due to shipping error of our company. All other refunds must be handled by the shipping company. Dorcas Dezines shall be available for telephone review and customer services, or emails of any matters related to this agreement during business hours. Dorcas Dezines will return telephones within 72 hours of business operation. Dorcas Dezines shall receive a flat fee of $75.00 (seventy five & no/100 dollars) plus reasonable out of pocket expenses to be paid via internet start-up cost for all web design, and half of all fees due before each job will be started. The balance of remaining payment is due two weeks after the job is started. A payment in full is due for any printed publications and balance due upon completion unless otherwise stated. Either party may terminate this agreement on 15 days notice for a material breach of the contract, or, 5 days notice without cause. Employer will not be refunded monies paid for start up cost and any out of pocket expenses and time devoted toward any given project at the time of cancellation. Dorcas Dezines may not advertise the fact of this consulting agreement without the prior consent of the EMPLOYER. Dorcas Dezines will abide by the privacy statement listed on website. Dorcas Dezines shall not provide services to any client which has not paid for services in full. All privileges will be cancelled and no monies will be return to Employer. Dorcas Dezines pledges to operate this business with the highest form of integrity and will place the need and concerns of our customers first.
